Please fill out the form below to register for FREE. A link will be sent to the email address you provide prior to the session. We will be using Zoom, an online video conferencing service, for our virtual sessions. Acknowledgement of the following release will be required. 

 

VIRTUAL ZOOM CLASS WAIVER AND RELEASE FOR ADULTS AND MINORS  

I do hereby release and hold harmless Destiny’s Daughters of Promise, its board members, and employees, facilitators or volunteers from any claims. I give permission for myself and or my child to participate in DDP’s online classes using the Zoom conferencing service. By completing your registration DDP’s virtual sessions, you acknowledge that you have read this document in its entirety, that you understand its contents, meaning, and effect, and that you have agreed to its terms completely and voluntarily. Destiny’s Daughters of Promise is relying on your consent to the terms of this agreement in its operation generally and in allowing you/your child, in particular to attend.

VIRTURAL ZOOM GUIDELINES * 

1. We will send you a link to the email address you entered to join the Zoom class meeting. 

2. The class meeting will be live and interactive. If the participant is under the age of 18 a parent or guardian must monitor the session.  

3. These sessions may be recorded and will NOT be available to the student for viewing afterwards unless otherwise agreed upon by the instructor. Recording the session may be used by Destiny’s Daughters of Promise on its social media platforms, newsletters and or training purposes only.  

4. Please review our “Participant Protocols” policy and if the participant is a minor, discuss responsible and appropriate behavioral expectations with your child that may be applicable online.  

5. During live sessions, remember that everything within the camera viewing area will be visible to the instructor and other students. Please make all household members aware of this. 

 

I have read and understood all of the above terms and conditions and, having enrolled myself and or my child in the virtual sessions offered by Destiny’s Daughters of Promise, agree to be bound by them.
